Forms
for travelin' Scouts
- Forms Maintenance Policy - The troop does NOT maintain forms. Parents should keep a copy of medical forms as many may be used over a two or three year period. Unfortunately, some camps require their own forms, so there is no guarantee that they can be reused. When going on a trip, please hand in a copy of the form for the leaders to maintain. At the end of the trip, the leader will try to get them back to you. If they can't get them back to you, they will be shredded.
- Troop 82 Medical Consent Form (.pdf
format) for all Troop Camps. This is our form and MUST be filled out prior
to a scout participating in any camping/touring activity. Hand it in to
one of the assistant scoutmasters when complete.
